This policy explains what personal information Jagmohan Multispeciality Hospital (“the hospital”) collects through this website, why, and what you can do about it. It covers the website only — medical records created during treatment are governed by separate hospital policies and the law.

What we collect

When you request an appointment or a call back, we collect:

• The patient’s name
• A mobile number
• The department, doctor and day you chose, if any
• Anything you write in the message box

Please don’t include detailed medical information in the message box. The doctor will ask what they need when they see you.

Why we collect it

Only to contact you about the request you made — to confirm or arrange an appointment, or to answer your question. We ask for your consent on each form before you submit it.

What we don’t do

• We don’t sell or rent your information.
• We don’t use it for advertising.
• We don’t share it outside the hospital, except with a service provider that delivers the request to the hospital on our behalf, or where the law requires.

How long we keep it

Retention period to be confirmed by the hospital.

Your rights

You can ask us to show you, correct, or delete the information you submitted, or withdraw your consent. Contact the grievance officer below.
